Does Cullman Urgent Care allow scheduling of appointments?
Urgent Care Center is a walk-in clinic. As such, it is not necessary to schedule an appointment. Patients are served on a first-come basis. Wait times will vary depending upon the number of patients.
What are our office hours?
The clinic is open Monday through Saturday from 8:00 a.m. until 7:30 p.m. We are also open on Sunday from 12:00 p.m. until 7:30 p.m. Normally, patients are allowed to check-in until 7:30 p.m. The office is open from 8:00 a.m. until 2:00 p.m. on the following holidays: New Year's Eve, New Year's Day, Memorial Day, the Fourth of July, Labor Day and Christmas Eve. We are open from 2:00 p.m. until 7:30 p.m. on Easter Sunday, 8:00 a.m. to 12:00 p.m. on Thanksgiving, and closed on Christmas Day.
What kind of paperwork can I expect to fill out?
Before your first visit, you will be asked to complete a Patient Registration Form and HIPAA Privacy Notice. This information will become a part of your medical record and will be held in the strictest confidence. Information in your record will be released to other physicians or to insurance carriers only with your written permission.
Each time you visit our office, you will need to sign in with the receptionist. Please report to her any changes of address, telephone number, marital status or insurance information so that your records can be kept up-to-date.
Please bring all your medications, or a list of all the medications you are taking, with you each time you come for a visit.
